Rapid Eye Movement Sleep Behavior Disorder

While sleeping, has it ever happened that someone near you started acting out their dreams? If they have a violent dream, do they start punching, kicking, and flailing their arms as if it was real? Furthermore, sometimes when you try to wake them up, it does not work, and they might or might not remember. Is such a situation indicative of something abnormal in the brain, or is it just the enactment of dreams? Can such behavior be diagnosed as a disorder? Can such behavior be injurious to you or someone else sleeping beside them? Is this because of some trauma, or has it a neurological basis?

What is Rapid Eye Movement Sleep Behavior Disorder?

Rapid eye movement sleep behavior disorder is characterized by the enactment of dreams and loss of normal REM sleep atonia. This one enacts what they were dreaming about but can be dangerous to oneself and others. This is usually linked with neurodegenerative diseases like Parkinson’s or Muscle Atrophy. Usually, the person can recall their dream after awakening. It is a parasomnia disorder, and strange voices and motor movements characterize it during sleeping. In this, the person starts to act out about what they were dreaming about, which can hurt themselves and others around them.

Symptoms

The most common symptom of REM sleep behavior disorder is insomnia. Most patients will fall off the bed and punch themselves or others. It can also occur with sleepwalking and other typical behaviors like flailing arms, voice enchantment, or punching. Some patients also reported hurting themselves during the episode. There is abnormal gross motor behavior, and in severe cases, there can be episodes of seizures. There can be confusion when waking up and decreased muscle tone during REM sleep. According to ICSD 3, the following symptoms should be present to be diagnosed with REM sleep behavior disorder −

The presence of R sleep in the absence of atonia

One or more of the following

Injurious or potentially injurious sleep-related disruptive behaviors;

The polysomnogram (PSG) revealed abnormal R behaviors.

Absence of epileptiform activity during R sleep unless RBD can be clearly distinguished from any concurrent R sleep-related seizure disorder;

Sleep disturbance is not better explained by another sleep disorder, medical or neurological disorder, mental disorder, medication use, or substance use disorder.

Diagnosis

The International Classification of Sleep Disorders, Third Edition (ICSD-3), which includes a history of repetitive dream enactment with complicated motor behaviors, as assessed during clinical history collection or observed during vPSG, is used to diagnose RBD. The first objective finding required for RBD diagnosis is skeletal muscle atonia during REM sleep1. In the absence of other illnesses that induce symptomatic or secondary RBD, iRBD is diagnosed.

Overnight vPSG should rule out additional sleep abnormalities with RBD-like characteristics, such as OSAS108, PLMS109, NREM parasomnias (such as sleepwalking, sleep terrors, and confusional arousals11, nightmares, and sleep-related seizures (mostly sleep-related hyper motor epilepsy)1). These diagnoses are checked for during the clinical examination but definitively ruled out during the night-time vPSG.

Etiology

This can occur due to the failure of spinal neurons during REM sleep. A link is established between RBD and neurodegenerative diseases like multiple muscle atrophy and Parkinson’s. In neurodegenerative diseases, there are lesions in Pons. Also, during neuroimaging studies, there has been an increase in dopaminergic agents, which can worsen RBD. Patients on antidepressants are also much more prone to REM sleep disorders. Withdrawal from antipsychotic medicines like benzodiazepines can also cause RBD.

Pathophysiology

Sleep consists of two types: rapid eye movement sleep and non-rapid eye movement sleep. In REM sleep, there are rapid eye movements, muscle atonia, which refers to not having mobility in our muscles, a desynchronized electroencephalogram, and dreaming. When one wakes up, many areas of the brain are alerted by the cerebral cortex. The areas that are activated upon waking up remain active during REM sleep. Disruptions in neurotransmission trigger RBD.

Risk factors

REM sleep behavior disorder was more prevalent in men than women. Risk factors include lower education level, low physical activity, diabetes, lower body mass index, head injury, and chronic olfactory and taste dysfunction. These might be the risk factors because these behaviors can affect certain areas associated with REM sleep, triggering the development of this sleep behavior disorder. Living alone can cause mortality in this sleep disorder, and this might be so. Living alone can cause psychosocial problems and stress, which might affect sleep.

Prognosis

Patients with RBD are at risk of developing cognitive impairment. They also have visuospatial and constructional abnormalities and altered visuospatial; they may have cognitive impairments and deficits such as verbal memory and executive function. There is also a cognitive decline in patients with this disorder. This can be because neurodegenerative diseases include cognitive impairments and deficits, which can worsen the prognosis and cause a decline in the prognosis.

Comorbid Disorders

CNS disease has been observed in people with REM sleep behavior disorder. Since a link has been established between neurodegenerative diseases, Parkinson’s disease is also observed. However, RBD symptoms precede Parkinson’s’ symptoms/ dementia can also be present after observed after RBD sleep behavior progresses. Multiple system atrophy was also seen. They also have psychiatric disorders like affective disorders, substance abuse, anxiety disorders, and personality disorders.

Post-traumatic stress disorder was also seen. Along with that, alcohol abuse was also observed.

In this disorder, the person enacts their dream while sleeping, which can injure themselves and others. It has also been linked to several neurodegenerative disorders, including Parkinson’s and dementia. Though it is manageable with medicines, without external help, it can worsen and be very difficult to control.

Virgin Galactic’s SpaceShipTwo crashes in engine test [Update]

Virgin Galactic’s SpaceShipTwo has crashed during today’s test flight, with the high-speed rocket plane coming down east of Mojave. Virgin Galactic first confirmed an incident had taken place via Twitter, with a terse message saying the space plane “has experienced an in-flight anomaly,” and promising additional information on the situation to follow. According to local reports, which also confirmed the crash, there has been one fatality and one injury, with SpaceShipTwo itself in pieces on the Californian desert.

Director of the Mojave Air and Space Port Stu Witt told the Kern Golden Empire News that a press conference would be held this afternoon. According to the site, scanner traffic suggested at least one fatality is involved.

SpaceShipTwo flies with two pilots onboard. ParabolicArc’s Doug Messier was on-site in Mojave, and reported on Twitter that he had seen the space plane encounter issues with its engines.

Today’s test was to be only the fourth powered flight for SpaceShipTwo, and indeed the first time since January that the engines were used.

It was also an opportunity for Virgin Galactic to test a new rubber-nitrous oxide engine which burns nylon from a polyamide-based fuel grain. Replacing the existing rubber-hybrid engine, the new system was expected to be more powerful and thus allow SpaceShipTwo to fly higher in the Earth’s atmosphere.

Virgin Galactic’s goal is to take space tourists to around 62 miles above the planet. At that altitude, the plane would officially be considered to be flying in space, traveling at speeds of up to 2,485 mph.

The crash follows the failure of the unmanned Antares rocket earlier this week, which exploded on takeoff as it began a resupply mission to the International Space Station.

Update: Richard Branson has said he is flying to Mojave to be with the team at the crash site.

Google’s John Mueller answers a mixed bag of SEO questions in rapid fire style in the latest video on the Google Search Central channel.

Previous editions of Ask Googlebot have focused on answering one question per video. But not every SEO question needs a four minute video in order to give a complete answer. Some can answered in a quick one or two sentences.

Here are five of those questions along with Mueller’s responses. There’s no theme to these questions, as they range from canonicals, to CDNs, to sitemaps files.

Let’s get into it.

Question 1: Canonicals

Q: Any chance a self referential canonical can override an existing canonical or preferred page? Are self canonicals really needed?

A: Mueller says site owners should avoid using multiple rel canonicals on the same page with they point to separate URLs.

Question 2: URLs

Q: Is it okay to put the month and date in URLs? Will this affect Google rankings?

A: Having the date in a URL is fine, Mueller says. It doesn’t affect crawling, indexing, or ranking of the page.

Question 3: Content Delivery Networks (CDNs)

Q: Is it a problem to use CDNs to deliver images? (Since the images are not on the site’s server and Google doesn’t support image canonicalization)

A: Using CDNs for images is fine. Mueller says images do not have to be on your server.

Question 4: Broad Audience Targeting

Q: What is the best way to appear in SERPs if a site is targeting a global audience with a single domain?

A: It’s fine to have a single domains for a worldwide audience, Mueller says. Many sites have just one version that’s globally available, and they are able to rank in multiple countries.

Question 5: Sitemap Files

Q: Is it a bad thing to leave the Google Webmasters verification file out of the sitemap.xml?

A: You don’t need to include the verification file in your sitemap file. Mueller says the verification file is just for Search Console. Nobody else can do anything useful with it.

Need even more of a breakdown? Let’s go by price:

Streaming video players! Yes, Apple TV is great (especially on sale), but what if you want to stream all of those ripped Blu-Rays sitting on the Synology in your basement? Bag of hurt! You’ll need something else. While a Chromecast at $30 is nice, it still doesn’t stream local files. Rokus and Google TVs work a little bit with the right apps, but I’ve actually had the best luck with a WD TV Live Media 1080P Player Wi-Fi which you’ll find for around $75. It also does most of the stuff that the other streamers do, but also has weather and apps you can set as an alarm clock. It is nice getting the weather in the morning on our bedroom TV.

Wait, back up. What is a ‘Synology‘ you ask? Only just the best NAS enclosure out there. You buy one of these starting at $200 (word to the wise, everyone who gets these loves them but wishes they got a bigger one) and not only do you have a great Time Machine compatible share but also a torrenting, VPN/Firewall Linux Machine with an easy-to-use GUI. The apps give you access to any type of media you are storing from any iOS or Android device. The $600 DS412+ 4 bay is a best bet.

If you have a new iPad Air like me, you are probably interested in a keyboard case. I’m in full agreement with Mark Gurman’s Smackdown. If you want thin, Logitech’s $80 Ultra Thin or Logitech Fabric Skin Keyboard Folio ($120) are great options. ZAGGkeys Folio $99 Black with Backlit Keyboard offers a great typing experience and backlighting. I also like Belkin’s $60 QODE line which are probably the most rugged of the bunch.

Overall and day to day I usually go with Logitech when it comes to iPad (of all sizes) keyboard cases. If you don’t need the case and just want a keyboard, check out Logitech’s EDU focused wired lineup (above). No Bluetooth tethering. Just plug and play and it works. Most reliable keyboard since Apple stopped making its keyboard dock.

Battery backup for iOS devices: This is an interesting area. I’ve reviewed huge items like the Hyperjuice that can even charge a Mac and things as small as the New Trent PowerPak that are great for using every day. Both are great at what they do, but lately I’ve been in love with a new little item: The RavPower ‘Lipstick’ flashlight charging solutions which run about $20. Basically, you get a very bright little flashlight and a full iPhone charge in something that looks like a maglight meets lipstick dispenser. Obviously great for the ladies’ purses as well.

DLP Projectors. I was recently given a great little DLP projector that I wanted to include in this roundup, but it kept turning off halfway through my videos. That doesn’t mean they are all bad. The category is a fantastic one and I have some old standbys to recommend. I love my portable Telstar (pictured above). Someday Apple will make something in this field.

Edifier – This is a great desktop Bluetooth speaker with mic that is great for conference calls. My wife won’t give it back because it sounds so great and looks just as nice.

Wren – I’m not usually a huge fan of AirPlay, but this beautiful Bamboo Speaker makes it work really well. Set and forget in your bedroom for great streaming of iTunes music. It sets up easily with an iPhone and can also do great direct line sound from a TV. Did I mention how good it looks?

Supertooth Disco Twin – A great Bluetooth option if you want some portability in desktop speakers. These sound great, can go for a few hours without power, and have great sound separation. They also look great.  I wouldn’t call these portable, but they do fill a niche.
